This section of the application form is designed to help you, and us, understand if you are eligible for this grant. It is important that you complete these questions before any others to ensure you do not waste your time applying for an unsuitable grant.

It is recommended that you contact the City of Canada Bay Grants Team on 9911 6555 or email grants@canadabay.nsw.gov.au to discuss your project and eligibility prior to submitting an application

Before proceeding, please confirm the following:

• you have read and understood the program guidelines 
• your organisation is not-for-profit 
• your organisation is incorporated, or is auspiced by an incorporated not-for-profit organisation for the purposes of this application
• your organisation is located in and/or supplies services to the City of Canada Bay. 
• your organisation is able to demonstrate financial viability 
• your organisation does not owe money to City of Canada Bay
• if your organisation involves adults working with children, your organisation has all the correct and current working with children's checks. 
• Your organisation has public liability insurance and it is attached below 
• Your organisation has acquitted previous grants from the City of Canada Bay Council and a copy of the most recent acquittal is attached below
